1       What is Scheme rate /MHR?

Answer: 

The Momentum Health Rate is the pricing guideline/average rate that Momentum Health sets for payment of claims. 

2       I received an account/bill from a doctor, what do I do with it?

Answer: 

Contact our Customer Care Department; make sure that you have the details of the account.
E-mail the account to cl.ingwe@momentum.co.za or member@momentumhealth.co.za
Visit our on-site consultants; please refer to the attached schedule for details.

3       How long do I have to submit an account/bill?

Answer:

Claims will only be considered for payment if they are submitted within 4 months from the date of service/treatment.
